Contract
You can call or e-mail our office to receive a hard copy of the Mail Service contract and Postal Service Form 1583, or simply download the copies provided below. Both Postal Form 1583 and the Escapees Mail Service contract are required to join our service. Please note that we are unable to accept faxed paperwork.

If you want a temporary mail account ranging from 2 months to 6 months of service, please download the following:

Temporary Contract and Postal Form 1583

• The Mail Service contract must be filled out completely and signed. Be sure to fill in all names (legal, nicknames, and initials) that mail may be addressed to and provide vehicle and emergency contact information.
• Postal Service Form 1583 is required by the United States Postal Service and gives Escapees Mail Service the right to act as your agent.
• The postal form must be filled out, signed, and notarized.

Identification
Two forms of identification are required. One piece must have a picture and a signature such as a driver license, passport, or a military I.D. card. The second piece must have your name, a number, and your signature. Examples are Golden Age Passport, AARP card or voter registration card. (Social Security cards and credit cards are not acceptable.)

Your completed contract, postal form, and copies of your identification pieces should be mailed (faxed applications cannot be accepted) to Escapees Mail Service, 101 Rainbow Drive, Livingston, TX 77399. (Remember, this is a corporate address and will not be the unique address we provide to you as a member.)
